b. The downloaded file has blank fields that you can fill-in via Microsoft Word.

c. After completing the form, save a copy to your computer.

d. Forward the registration form to MTSA by attaching a copy of the file to an email. Send the email with attachement to jcousineau@magictouchsoccer.com

e. You will receive a confirmation email from MTSA when your registration form has been received.

If you are unable to download a Word document, you can download a pdf file. Complete the form by hand and forward it via US Postal Service to MTSA (address listed on the form)

Be sure to include an email address as MTSA will use email for all correspondance.

a. MTSA will hold your child's camp spot for one week after receiving the registration form via email. Full tuition is required to complete registration process.

b. MTSA Tuition is $90 per player, per week. MTSA does offer the following three discounts:

Early Registration - Save $10 by registering and paying tuition prior to May 31st.

Multiple Weeks - Players who attend more than one week of camp receive a $10 discount for each additional week of camp they attend.

Multiple Siblings - Families who have mulitple siblings attending the same camp week, receive a $10 discount for each sibling after the first registered child.

In the registration confirmation email, your child's camp tuition bill will be listed and all other important information will be highlighted.

c. Forward the tuition check (payable to Magic Touch Soccer Academy) via the US Postal Service to:
